Measure the cornea first, book the flight home second: LASEK in Korea

LASEK in Korea suits eyes where a flap is a poor idea, such as thinner or borderline corneas, but the surface needs three to four days under a bandage lens, so exam numbers and flight date decide it together.

After LASEK, a soft bandage lens sits on each eye for the first three or four days while the surface layer grows back, and those days reshape a trip to Seoul more than the laser does. So LASEK in Korea is planned around healing days as much as around the surgery date.

Whether it's the right choice for you at all comes down to three numbers from your own exam: corneal thickness, the shape on your topography map, and the size of your prescription. The fourth is the date on your flight booking.

If you've spent a few weeks on r/lasik, you've probably seen LASEK called "the conservative one" and "the painful one" in the same thread. Both labels have something behind them. Below is what each number means, how it pushes the decision toward or away from surface laser, and what the first week looks like when you're staying in a hotel instead of your own bed.

Why is LASEK still offered when faster options exist?

LASEK reshapes the cornea without cutting a flap. That one difference is the whole reason it survives next to LASIK and SMILE LASIK: no flap means no flap to dislocate and, according to the American Academy of Ophthalmology's EyeWiki entry on LASEK, a lower risk of ectasia (a gradual weakening and bulging of the cornea).

The procedure has four steps. A dilute alcohol solution loosens the thin surface layer, the epithelium. That layer is moved aside, the excimer laser reshapes the tissue underneath, and the layer is laid back over the treated area with a soft contact lens on top as a bandage. At Eyeonce Eye Clinic the excimer laser is the WaveLight EX500.

The trade-off is time. The same EyeWiki entry lists postoperative pain, discomfort and slower visual rehabilitation among the disadvantages. If you want to compare the flap and small-incision routes side by side, the posts on what to check before LASIK in Korea and how SMILE LASIK differs cover those.

Corneal thickness, and what the laser leaves behind

Thickness on its own decides less than people expect; what counts is the subtraction. The laser removes tissue, a larger prescription removes more, and the question is whether enough untouched cornea remains to carry the eye's internal pressure for the rest of your life.

With LASIK, the flap uses up part of that thickness without adding strength, so the load-bearing layer is thinner than the total suggests. Surface laser leaves no flap, which is why a cornea that looks borderline for LASIK can sometimes still be considered for LASEK. EyeWiki notes that in one early LASEK series the authors could treat a larger ablation zone in thinner corneas without flap problems.

A thin cornea can be thin because that's simply how it was built, or because it's an early sign of a weaker cornea. Pachymetry gives the figure; it does not tell you which of those two you have.

Topography: the shape thickness can't show

Shape. A topography map shows how evenly the cornea curves across its surface, and a cornea of normal thickness can still show uneven steepening, often in the lower half, that hints at keratoconus or a cornea prone to it. When that pattern appears, any laser that removes tissue, flap or no flap, may be the wrong answer.

At Eyeonce, these measurements are taken on Topcon diagnostic equipment, and Dr. Baek Dong-won handles the diagnosis, the surgery and the follow-up personally, so the person reading the map is the person who would operate.

A thin reading by itself doesn't make me say no. What I look at is whether the two eyes are roughly symmetrical, whether the steeper area is centered or drifting downward, and whether your prescription has stayed stable over the last few years. If those signals point the wrong way, the next step is to look at lens-based options such as the EVO Aqua ICL, or about not operating at all.

Prescription size and surface laser

LASEK is studied mostly in low to moderate myopia. In that range, the research summarized on EyeWiki finds LASEK, PRK and LASIK broadly similar in predictability, efficacy and safety.

Higher prescriptions change the picture. In one study in high myopia summarized on EyeWiki, 14.3% of LASEK eyes lost more than one line of vision compared with 1.2% of LASIK eyes. That is one study, not a rule, but it fits the general reasoning: a deeper treatment on the surface means more tissue to heal and more room for haze or regression. Astigmatism adds to the depth as well.

So if your glasses read somewhere around minus 8 or beyond, or you have a large astigmatism component, it's worth reading how high myopia and astigmatism are assessed together before you settle on surface laser. The exam may still land on LASEK; it may land on ICL instead.

How many days in Seoul does LASEK need?

Plan on roughly four days in Seoul after LASEK before you think about the flight home. EyeWiki describes the bandage lens being removed once the surface has fully regrown, "typically 3 or 4 days post op", with antibiotic and steroid drops for the first days and a milder steroid continued for up to a month in one common technique.

Point after surgeryWhat is usually happeningWhat it means on a trip
Day 0Bandage lens placed, drops startGo straight to the hotel; screens and sightseeing can wait
Days 1โ€“2Surface regrowing; discomfort, tearing and light sensitivity are commonPlan indoor rest near the clinic
Days 3โ€“4Bandage lens checked and removed if healedThis visit has to fall on a day the clinic is open
Weeks 1โ€“4Drops continue, vision sharpens graduallyUsually managed at home, with drops packed in carry-on

The clinic's hours matter more for LASEK than for flap procedures. Eyeonce is open Monday, Tuesday, Thursday and Friday 09:00โ€“18:00 and Saturday 09:00โ€“16:00, with lunch from 13:00 to 14:00, and is closed on Wednesdays and Sundays. A Monday surgery puts day 3 on a Thursday. A Saturday surgery puts day 3 on Tuesday and day 4 on a closed Wednesday, which is the kind of clash to spot before you book anything. Appointments are required.

Two more dates sit before the surgery itself. The U.S. Food and Drug Administration's guidance for LASIK candidates says to stop soft lenses for 2 weeks before the initial evaluation, toric soft or rigid gas permeable lenses for at least 3 weeks, and hard lenses for at least 4 weeks; the same logic applies to surface laser measurements.

The clinic, in the Duo Sinnonhyeon building at 368 Sapyeong-daero, Seocho-gu, lists taxi, AREX-plus-subway and limousine bus routes from Incheon Airport on its website, which helps when planning arrival day.

What are the risks of LASEK, and how often do they happen?

Most problems after LASEK sit in the first week and come from a healing surface; a smaller number show up later as haze or a change in prescription. The list below uses the figures EyeWiki reports where it gives them.

None of these can be ruled out in advance for a specific eye. What the exam can do is identify eyes where the odds look worse, such as an irregular topography, an unstable prescription or significant existing dryness, and steer them elsewhere.

Dry eye is the part of refractive surgery I think about most, partly because I co-authored the Textbook of Dry Eye (2019). Someone who already struggles with contact lenses by late afternoon deserves a tear assessment before any laser decision, not after. The complication I take most seriously in the first days of LASEK is infection under the bandage lens, because it is uncommon but moves quickly, so any sharp increase in pain or redness is a reason to come back the same day.

Who checks your eyes once you've flown home?

Your own ophthalmologist does the routine checks at home. The FDA advises regular follow-up for at least the first six months after LASIK, and a surface treatment needs at least that much attention while haze risk and drop schedules play out.

Eyeonce offers Lifetime Aftercare: re-examination whenever you return, with retreatment support if it is needed; the specific conditions are confirmed at consultation. Before you leave Seoul, ask for a copy of your surgery record and drop schedule to hand to your doctor at home.

Dr. Baek's background (former Clinical Instructor, Dept. of Ophthalmology, Samsung Seoul Hospital; member of the Korean Society of Cataract and Refractive Surgery and the American Society of Cataract and Refractive Surgery) is listed on the doctor's page if your home ophthalmologist wants to know who did the surgery. For the wider picture of planning eye surgery here, the South Korea eye surgery overview is the place to start, and the Eyeonce Eye Clinic home page lists every procedure offered.

If you're weighing LASEK and already have a rough window for the trip, send the dates you're considering and a photo of your current glasses prescription on WhatsApp, where consultations are offered in English, and lay the dates against the clinic's opening days in the table above. The in-person exam then decides whether surface laser fits your corneas or whether another option makes more sense.

Results and suitability vary by individual. Your procedure recommendation is confirmed only through an in-person diagnostic exam.

Lasek in korea โ€” questions & answers

Usually the first few days are more uncomfortable, because the surface layer has to regrow under a bandage lens. In one early LASEK series cited on the AAO's EyeWiki, 62.7% of patients reported no postoperative pain, so roughly a third did feel it. Prescribed drops and rest manage most of that discomfort.
Plan to stay until the bandage lens has been checked and removed, which is typically on day 3 or 4. The doctor decides at that visit whether the surface has healed enough for travel. Booking the flight before that check is the scheduling mistake to avoid.
Both are surface laser treatments with no flap. The difference is that LASEK loosens the epithelium with dilute alcohol and lays it back after the laser, while PRK removes it. Studies summarized on EyeWiki find them broadly similar in low to moderate myopia.
The excimer laser can treat astigmatism together with myopia within limits. A larger astigmatism adds to the treatment depth, so it is weighed alongside corneal thickness and topography. The in-person exam decides whether surface laser or a lens-based option suits the combination.
Some do, because there is no flap that could be displaced by a blow to the eye. That benefit has to be balanced against the slower first week. It becomes one factor in the decision, next to the corneal measurements.
